Your body gets scanned and cloned with the implant installed, and your consciousness is tranferred to that clone for the first time, killing your old body. After that, capsuleers have to be in their pods in order for their consciousness transfer to work. When the pod detects a breach in it's structure, the implant activates and makes a burst scan of your brain to be copied into the new clone. This scan also kills you instantly, and you wake up in the clone reanimation unit back in the station where you selected your medical clone.
There is also the possibility of jump-cloning. You enter a clone vat and your brain gets scanned with a sllwer method, but without destroying your brain in the process, and your consciousness is transferred to another clone in some other station. Your previous body is still kept alive in the clone vat while you are in your jump clone. It is also possible for your jumpclone to have different atteibute boosting implants and looks, and supposedly an explanation to alts within EVE too, as this happened in the EVE: True Stories comic book. I'd take that with a grain of salt though.
And then there's the traditional "soft cloning" which is accessible to everyone, even the lowliest of baseliner, provided they have the ISK. Your brain gets scanned within a lab and a clone is created with the scanned brain. When your body dies, this clone is activated. The drawback to this is that you will always have a gap in your memory from the point when your brain got last scanned, making the tech inferior to the tech capsuleers use.
Finally there's us with our sleeper implant. We still don't know the specifics of how it works(unless EVE: Source has information about it, still need to get that book), but what is generally accepted that this implant can transfer to another clone instantly without the need for special equipment, even at the moment of death at any location. This is why the tech is superior to the capsuleer cloning tech, as they are forced to be in their pods to have their memory up-to-date when they die.
